10 中药芪苈强心胶囊
芪苈强心胶囊主要以黄芪、附子、红花、葶苈子、泽泻等药物组成,全方以益气温阳药治其病本,辅以活血通络药,使气旺血行络通,阻断血瘀络阻的病理中心环节,兼用利水消肿药以治其标。从中医络病学说探讨,心气虚乏、运血无力是慢性心力衰竭发生的中医病机之本,而血运无力、络脉瘀阻则为其病机之中心环节[13 ] 。络脉瘀阻日久,其末端进行的津血互换障碍,过多的津液不能回流聚于络外而发为水肿。这与西医学神经内分泌系统激活为心力衰竭发生的早期表现,神经内分泌系统参与的心室重构是其基本机制的新概念相似。结合该药药理研究,芪苈强心胶囊既能增加心肌收缩力、增加心排血量、增加肾血流量、利尿消肿,改善血流动力学,缓解心力衰竭症状,具有传统强心、利尿、扩血管作用,又能明显抑制肾素2血管紧张素2醛固酮系统,降低Ang Ⅱ、抑制醛固酮升高,明显阻抑室壁厚度的增加,增加心脏指数及减少心室重塑,从而改善心力衰竭的生物学基础。全方标本兼治,从多途径,多环节,多靶点治疗心力衰竭,体现复方中成药在治疗心力衰竭方面综合优势。
