术后功能性胃排空障碍研究进展

4 　预　　防

预防重于治疗,应及早识别与术后FDGE 相关的危险因素,并在术前尽可能予以纠正,积极防治术后并发症,合理选择手术方式。何建苗等^{[21 ]} 提出胃肠道重建过程中,应尽量符合生理解剖状态,才能使胃肠道运动协调一致,有望使部分患者避免术后发生FDGE。王奎海等^{[22 ]}报道,在残胃空肠行毕Ⅱ吻合基础上,加做Braun 吻合的120 例,仅发生胃瘫1例,发病率为0. 83 %;未做Braun 吻合的360 例,发生胃瘫19例,发病率为5. 28 %。认为Braun 吻合在残胃空肠吻合的基础上形成“代胃”结构,在解剖上能接近正常的解剖结构,对于术后的胃肠运动有很好的协调作用,胃的移动性慢波及输出袢空肠的逆蠕动通过Braun 吻合的存在得以消除。十二指肠内的碱性消化液在进入输入袢空肠时,由于有Braun 吻合的存在,大部分消化液通过Braun 吻合口进入输出袢,起到了很好的分流作用。
