2. 3 抗炎与调节神经激素活性作用
炎症与心衰的发生发展密切相关,他汀类药物主要通过降低炎症因子和抑制粘贴分子而发挥抗炎作用,以减慢心衰的发展,抗炎作用也可与其上调内皮NO 产物和抑制超氧化物的释放有关。
神经激素的激活在心衰的发生发展中占有重要地位,抑制血管紧张素2醛固酮系统可改善心衰患者预后,他汀类药物可以减少血管紧张素受体I 数量,并提高血管紧张素受体拮抗剂的作用[ 8 ] 。
此外,他汀类药物还可提高循环中来自骨髓的内皮祖细胞的数量,通过促进循环中内皮祖细胞的增殖、迁移、生存能力而诱导新生血管形成,促进内皮修复[ 9 ] 。
临床对舒张性心衰和收缩性心衰的治疗方法有所不同,他汀类药物对前者发挥的有益作用,对于下一步更加深入细致的研究有启示作用。
